dashboard) while the real work silently failed, or ran with broader
credentials than it needed.
(a) Ungated destructive action.
An agent/automation deleted, dropped, wiped, or overwrote something
it should have asked before touching.
(b) Unverified fact asserted publicly.
An automation confirmed, cited, or reported something as true that
turned out to be hallucinated or fabricated.
(d) Quietly went dark behind a green light.
Everything looked fine on the dashboard while the process behind it
had stopped doing real work, silently, with nothing to flag it.
